USPh provides a wide array of Employee Benefits for its employees*:
Medical Insurance Plans
USPh offers its employees a comprehensive medical package to fit their needs.
Employees are eligible to participate on the first day of the month following
30 days of employment. Dependent coverage is also available. The plan premiums
are paid by both employee and USPh contributions.
Dental Insurance Plan
USPh offers a comprehensive dental coverage that begins on the first day of the
month following 30 days of employment. The plan provides comprehensive dental
coverage, and is also available for your dependents. The plan premiums are paid
by both employee and USPh contributions.
Vision Plan
USPh offers an optional vision plan that includes coverage for vision
examinations, lenses (standard or contact) and frames. Dependent coverage is
also available. The plan premiums are paid by the employee.
Basic Life and AD&D Insurance Plans
Basic Term Life Insurance and AD&D insurance are provided to employees. The
plan premiums are paid by USPh.
Optional Life and AD&D Insurance Plans
Additional Life Insurance is available in $10,000 increments for our employees
and $5,000 increments for spouses. The additional Life Insurance coverages
include group term life insurance, dependent life insurance and accidental
death and dismemberment accident insurance. The plan premiums are paid by the
employee.
Disability Insurance Plan
USPh offers optional Short Term Disability, paid by the employee, and a company
sponsored Long Term Disability benefit plan, paid by USPh. The plans provide
partial income replacement for employees who are unable to work due to illness,
pregnancy or injury.
Flexible Benefit Plans
These plans allow employees to set money aside through payroll deductions to
pay for certain qualified medical and dependent care expenses on a pre-tax
basis. An optional plan credit card is available to make payments convenient.
401(k) Plan
USPh sponsors a qualified 401(k) Plan. The plan allows employees to contribute
a portion of their pay on a pre-tax basis into their own account within the
retirement plan. Investment options include a diverse group of mutual funds.
Continuing Education
USPh provides educational assistance to all licensed clinical employees.
Paid Time Off ("PTO") Plan
USPh provides a competitive PTO plan in addition to granting certain recognized
holiday time off. The PTO plan commences after the employee's introductory
period.
* Employees are eligible to participate in the plans on the first of the
month following 30 days of employment. In order to participate in the plans you
must be a full-time employee or work a minimum of 30 hours per week. The 401k
plan does allow all employees to participate.
